The cardiovascular health and rehabilitation service at UCLH is made up of a multidisciplinary team of cardiac nurses, health professionals and exercise specialists who can provide advice and rehabilitation to patients with heart disease.
Patients are referred to this service via Barts Heart Centre and may be referred following recent cardiac surgery, percutaneous coronary intervention, myocardial infarction, implantable cardiac defibrillator or diagnosis of angina or heart failure.
Patients are offered an individually tailored rehabilitation programme which incorporates education, exercise, psychological support and behaviour change through a self-management approach. The aim is to empower patients with the knowledge, belief and goal-setting skills to make and maintain lifestyle change.
All patients are advised on the safe principles of exercise and how to rate and pace activities. They are offered a six-week programme of clinically supervised exercise, followed by community exercise classes, as appropriate, through a joint initiative with the Camden Physical Activity Health team.

Appropriate cardiac patients may be visited while inpatients at University College Hospital by a nurse specialist from the cardiovascular health team.
Kết quả xổ số Quảng NamAll patients are offered an outpatient consultation with a cardiovascular nurse specialist followed by either a comprehensive, weekly self-management and exercise rehabilitation programme for up to six weeks, or advice on an independent home programme if preferred.
All patients are reviewed on completion of their cardiovascular health programme at three months and one year.
